The embedded pharmacist for primary care practices and health plans

Lodestar Collaborative Care brings comprehensive medication management (CMM) into your practice. Let us resolve medication problems so your providers are supported, patients are healthier, and the clinic meets a higher standard of care.

Our Approach

We believe comprehensive medication management is the most thorough and relationship-focused method to optimize a patient's medications. We follow a defined five-step process the same way for every patient, not improvised visit to visit.

To make our collaboration seamless, our pharmacist is embedded into your clinic's workflow and EHR, so our work is transparent and your patients see that we are an extension of your care.
