Why Lodestar Collaborative Care exists
We started Lodestar Collaborative Care because pharmacists are an integral part of advancing primary care, yet their expertise is not easily accessible for independent practices. We hope to contribute to building a sustainable infrastructure for every organization striving to achieve truly team-based, comprehensive care for their patients.
We focus on the Pacific Northwest, particularly rural and underserved communities where clinical pharmacy access is most limited and the potential impact is greatest. We understand the realities here: geographic isolation, workforce constraints, complex patient needs, and the pressure to deliver more with less. Our partnership model is designed to work within them.
